Champlain LHIN Accomplishments 2010-11
This year’s highlights fall under each of the LHIN’s priorities outlined in our strategic Integrated Health Service Plan 2010-2013.

LHIN Update - Assisted-living services
Seniors in the Champlain region are benefitting from a new assisted-living program that helps them maintain their independence. Funded by the Champlain LHIN and operated by various community agencies, services include homemaking, personal hygiene, care coordination, security checks, and a 24/7 on-call service.

2011-2012 Annual Business Plan
The plan outlines the goals and objectives we will be working to achieve this coming fiscal year as we move towards our vision of creating healthy and caring communities today and for the future and support the Ministry of Health and Long-Term Care in the attainment of provincial priorities.

Community Engagement Strategy 2011-12
This document includes the Community Engagement Framework and outlines the Champlain LHIN’s Annual Community Engagement Plan 2011-12. The purpose of this plan is to present engagement activities that the LHIN will carry out during the year that will contribute to health system improvements.

Summary of Senior Friendly Care in Champlain LHIN Hospitals
Building upon formative work initiated in Champlain LHIN, a Senior Friendly Hospital Strategy has been endorsed by the Ministry of Health and Long Term Care and the LHINs across the province. The first phase, an analysis of Senior Friendly Care, is intended to promote awareness of senior friendly hospital care, provide a baseline of current activity, and identify promising practices intended to improve the health status of hospitalized seniors in Ontario.
